Ok so this is a wierd one. It doesn't happen often thankfully, but in certain situations my character will just keel over and die, and I don't know why. It's happened about 3-4 times now, (and just now).

This time I was walking over the edge of a roof (no it wasn;t the fall, I didn't even get to fall and it wasn't far down anyway) as soon as I stepped over....dead. Maybe I hit something and there's a really harsh havoc system like there is in Bethesda titles, if you hit something at a certain speed it kills you, just like the cars in FO4, too fast and at a certain angle and you run the risk of falling foul of havoc.

I know what you mean, there are some pipes in the badlands and if you try to climb them its insta kill for you.
Its certain actions that maybe get you in a weird position that always insta kill you, even small jumps onto weirdly angled stuff can kill you.
If you jump on a rock and later instead jumping back down you just fall, you die.
Most likely culprit is physics intersection.... game treats it like a fall or crush.

I've gotten hit with it a couple of times, mostly in corners between separate objects, or when jumping on trash piles near walls. I've learned to avoid both if I can.

Physics do get crazy sometimes..

Climbing in a windows get shot out of the window out of Night city to the edge of the map, The fact the character lived and I was like 4km away from the window was amazing.
I also had this happen, mostly on pipes. Another ridiculous physics interaction is that my character keeps tripping over bodies lying on the ground. Because of buggy camera position you do not know you have tripped, because all that you see is you screen going fuzzy and you are stuck in place for a while and have no idea why. From watching NPCs walking over bodies and seeing them faceplant, I can only assume that also happens to the player.
It happens when you are on a slope. I don't know why, but on some slopes you'll gain massive amounts of speed and accelerate way too far and the game seemingly calculates it as "distance" for fall damage. If that happens you just die from stepping from the sidewalk onto the street.
Glad I'm not the only one. Some good explainations as well. Yeah it did feel like a crush, like I got crushed between a satellite dish on the roof and something else, garbage bag maybe. And I've also had the distance fall damage as a result of acceleration on a slope.
