Why is the game so laggy for me now? The mouse is slow when I'm in-game, but when I'm at the main menu it's fine. Ever since 1.3 my game has lagged terribly, and it's too hard to play the game now. I could play the game on Ultra spec without any terrible lag, but now changing my spec from ultra to high/medium doesn't even seem to help. Will another patch fix this problem?

Steam? They say steam with 1.3 is like that.
If so, you need to reinstall and only patch up to version 1.2 until they fix it.
1.3 does seem to have impacted my system as well. Game was running smoother on 1.2 for me.
No I don't use Steam. I have the boxed version of the game and yet it still lags...
So I should just uninstall it then reinstall it again to fix it? And not update it to 1.3? How do I do that? I thought it said it will automatically update to 1.3 if you reinstall it.
I have had similar experiences. The game is a bit more choppy now on 1.3. It's not horrible but every 5-10 seconds there is a second of lag which can take away from the fluidity of things. My friend has also been forced to play on low setting from medium after patch 1.3
I don't know about boxed versions, I have the GoG version. I downloaded all the patches from here and patched manually. I never even had to unplug the net. One annoying thing: the launcher keeps popping up and you have to close it after every patch or you get errors.

I think they are a bit stumbling in the dark with the performance fixes - I have higher FPS in some places and new loading events in others. Overall it runs fine for me, but it's also still crashing regularly.

At this point, I cannot finish the damn game. The cutscenes became so laggy that they just skip to the end right after they start. But for the final dragon battle this is a disaster because I cannot make the necessary mouse-clicks to progress!
I've made changes to depth of field options, v-sync, motion blur and the best I get is it being so choppy at the beginning of the scene, it just keeps stuttering and looping over and over and not going forward.

All this started after updating to 1.3 of course. Never had a single hiccup with the cutscenes before.

The cutscenes lag the fuck out, like down to 20 FPS for me. Gameplay is 50-60, then any kind of cutscene kills the FPS. I've got the GOG version, and it only hapenned after 1.3.

Same here-- I'm using the GOG version.
Might be just me, but do the cut scenes now have more elements on screen than before!. Like increased distance rendering and more effects!, might be just a placebo effect on me..but I think I am seeing changes in the start of the game itself (or may be I didn't notice them during my 200+ hours spend on the game).
